Employment-Protection From Harm and Investigation Coordinator

 

Job Description:

Reports To:           Executive Director

Primary Function: To Fulfill the Duties of the Protection From Harm Coordinator and to coordinate and complete Investigations.

Responsibilities:

1.         Fulfill the duties of the Protection From Harm Coordinator.
a. Monitor Daily Reports (electronic and paper)
b. Coordinate Bi-weekly Protection From Harm Meeting and Complete Tracking and Minutes
c. Complete and Submit Reportable Incidents
d. Ensure Follow Up Documentation
e. Notify DIDD of Required Reports
f. Complete Internal Investigations for ICF Programs
g. Ensure Monthly Agency PFH Training
h. Complete Annual Trend Study
i. Chair Safety Committee

2.         To complete the requirements of the Human Rights, Restrictions and Consent process.
a. To coordinate the ICF HRC meetings
b. To obtain written consents for restrictions, medical procedures and psychotropic medications in ICF
c. To complete the requirements for the Medicaid Waiver programs regarding restrictions and psychotropic medications.

3.         To monitor, review, update and train on the Emergency Preparedness Plan. To coordinate drills to ensure agency participation in a variety of drills.

4.         To work with the management team to ensure daily function of the agency.

5.         Assist the Management Team with Special Events

6.         To serve as the agency HIPAA Privacy Contact.

7.         Assist the Executive Director.

8.         Perform other duties as assigned.

Qualifications:

Bachelors Degree or relevant experience in Human Services area.

Ability to be insured as a vehicle driver by agency’s insurance program.

A valid driver’s license.

Ability to write investigative reports, incident reports and corrective action reports.

Good knowledge of basic English sufficient to read instructions, keep records and write reports.

Successful completion of background and reference check.

Successful completion of a drug screen.

Ability to perform the essential functions and activities of the job with or without reasonable accommodations.
